[REVIEW] Enlighten + PicoSure = Two Is Better Than One

Earlier this year, I was invited to try out the PicoSure laser at Dr Joseph Yang’s Aesthetics and Medical Clinic in Wisma Atria. If you had read my earlier review on PicoSure laser, you would have known by now that it is one of the latest generation of aesthetic laser which administers laser pulses in picosecond (trillionth of a second!).

Unlike the previous generation of nanosecond technology lasers which utilise photothermal (heat) energy, picosecond lasers use photomechanical impact (light pulses) to directly target skin concerns without causing thermal damage to surrounding tissue. They are much more powerful and safer with very little or almost zero downtime. And for that reason, they are gaining popularity among the aesthetic doctors in Singapore.

Dr Yang was the FIRST doctor in Singapore to own a PicoSure Laser, and his PicoSure Laser treatment has recently won him the ‘Style: Best Beauty Treats 2016’ award by Style Magazine.

Constantly seeking betterment of his practice of an aesthetic doctor, Dr Yang is always on the lookout for aesthetic medical equipment for his patients. When he saw that there is another high-performance Picolaser (known as Enlighten) that is tunable and highly customisable to treat different skin concerns, he knew he had to get it. So now, he did it once again by being the FIRST and ONLY doctor in Singapore to own BOTH the PicoSure laser and Enlighten laser.

Dr Yang was very kind to invite me for another round of laser treatments.

What is Enlighten Laser?

Enlighten laser is the world’s first dual wavelength (1064 nm & 532 nm) and dual pulse duration (both picosecond & nanosecond) laser system featuring class-leading treatment parameters and customization. It is also said to be suitable for all skin types.


Combining the benefits of two highly effective wavelengths in ultra-short pulses to selectively target and shatter pigmentations, while at the same time cause a non-thermal, laser induced shockwave disruption and remodelling to the upper dermis. What we get is a firmer, brighter, more even complexion in as few as 1-2 treatments. And best of all, there is no downtime.

Treatment

Dr Yang’s assistant, Angela, was all smiles and was expecting me when I arrived at the clinic. Was offered a cup of warm water before being brought to the treatment bed where I was made to lay down for “face-cooling” prior to the laser treatment.

My face was cleansed before cold towels were placed over my entire face for about 15-20 minutes to bring down the temperature of my skin as well as to achieve some numbing effect. But having done PicoSure laser treatments with Dr Yang previously, I know I’m in good hands and there is nothing to be terrified of.

Once my face was ready, the jovial Dr Yang came into the treatment room with a loud “Good morning!” when it was late afternoon. LOL! I’m always happy to see Dr Yang and Angela. They always make me feel welcomed and at ease.

Two is Better than One

Dr Yang took a brief look at my face and told me that he’s going to administer BOTH Enlighten and PicoSure lasers on my face. Thinking out loud, I mumbled, “can ah?” and Dr Yang overheard and answered “CAN! Why not?” LOL!

He explained that the longer 1064nm wavelength of the Enlighten laser can be used for skin rejuvenation, laser toning (for a more even and radiant complexion) as well as to tackle pigments which are embedded deep beyond the epidermal layer of the skin. For more superficial pigments like sun spots, using shorter pulses (of 755 nm wavelength) of PicoSure laser will yield optimum results.

Dr Yang started the treatment first with Enlighten laser. He told me “eyes close, whatever you do don’t open your eyes” before proceeding to give a “trial shot” on my lower cheek to test my comfort level. I couldn’t be sure if it’s due to my high threshold of pain or that the laser is really gentle, there was NO discomfort at all. If I had to compare it with IPL, I think this is about the same or less painful than an intensive IPL session.

After completing the first round of shots and seeing that I could tolerate the strength of the laser pretty well, Dr Yang increased the strength for the next round, focusing on the nose area to remove blackheads and undereye area to eliminate dark eye rings. Again, I would say the discomfort/heat is still bearable. On a scale of 1-10, I would rate it as 3.5 or maybe 4 when focusing on the nose area?

After we are done with Enlighten laser, Dr Yang proceeded right away with PicoSure laser. Since I’m not new to PicoSure laser and it’s not my first time, Dr Yang used slightly higher strength than my previous treatment to treat the stubborn (yet not so dark) spots on my upper cheek area.

All was done in a short amount of time, say 20 minutes at most? And that’s like a couple thousands of shots administered and almost no downtime at all, except for some redness and a little bumpiness on certain parts of my face.

Angela proceeded to cool down my face with cold towels for another 20 minutes. After that, I was good to go.

Headed straight to the toilet and took a selfie with my no makeup face. Not kidding you right? No obvious redness or blotchiness. There is almost zero downtime and I could just apply some lipstick and head out for dinner or some shopping right away.

Post-treatment observations

My skin was already glowing (blushing pink complexion) but felt a little tight and dry right after the laser treatment. So what I usually do is to apply mask on that same night and do the same diligently for the next 2-3 nights so as to replenish any moisture loss. Based on my personal experience, there are 2 very important things you need to do after a laser treatment – Apply masks and sun protection.

I did notice that the stubborn spots on my upper cheeks turned slightly darker right after the laser treatment but become lighter after a few days.

The fine lines are less visible and my skin is more plumped up, which in turn makes my nasolabial folds (laughing lines) less obvious too.

The pores (on my nose and areas next to the nose) appear to be more refined and blackheads also diminished.

I have in fact gone for another session of the Enlighten + PicoSure lasers treatment and my skin just keeps getting better.

    After reading your reviews on PicoSure laser, am thinking of trying it out myself. I have light pigmentation, that is not very visible. You reckon PicoSure should suffice?

    Another very important question… you mentioned zero downtime. I can have this treatment before Christmas?

      Hello Mety,

      With regard to your question on downtime, whether there is any downtime or not will very much depend on the treatment required/rendered. If what you want to achieve is to remove, say for example, freckles, then of course there will be downtime. Cos for removal of freckles, the laser will focus on the spots rather than the entire face, and the spots will turn red and then darken over the next few days before scabbing and falling off to reveal new, clear skin. Similar for tattoo removal, there will be downtime for sure.

      For myself, the laser treatment I did was to achieve a more firm, even-toned and radiant complexion so there wasn’t any downtime. So if what you want to achieve from the said laser treatment is that, then I can almost safely say it’s zero downtime. Maybe some redness (more common for people with sensitive skin) immediately after the laser session, but the skin will be back to normal after a while. If you want to have glowing skin for Xmas, I would suggest you book an appointment and do the laser at least 3-4 days beforehand. Then apply mask diligently every night (or at least alternate nights) to give your face plenty of hydration (as your skin might feel tight and dry after the laser treatment) so that you can look your best on Xmas day :)

      As for whether PicoSure itself will suffice, I can’t really comment on that. You might want to consult Dr Yang as I think he might be the best person to advise after the consultation.
