An Interview Session On The latest THERMAGE® TOTAL TIP – A Non-Invasive Skin Tightening Procedure


In the past, the only significant option available to look 10 years younger was a surgical facelift, which results in several weeks of recovery and pain. However, surgical facelift is not guaranteed to last more than 5 years. And with any surgical procedure, there are always potential side effects and risks involved too.

Today, Thermage is the ultimate option. Thermage is a non-surgical, skin tightening procedure that can be performed on the face, eyes, neck and body with little to no downtime and little pain.

Thermage uses radiofrequency technology to non-invasively help smooth, and contour the skin, as well as temporarily reduce the appearance of cellulite, in a single treatment with little to no downtime. The Thermage Total Tip is effective for facial treatments because it delivers uniform, volumetric bulk heating. This heat is dispersed through the vascular structures, which are more abundant on the face than other body areas. The dispersal and uniformity of energy may lead to increased patient tolerability and allows practitioners to deliver an efficient volume of heat on the face, with highly positive outcomes. Last week, I was delighted to be presented an interview opportunity with Mr Jeff Nardoci, Vice President of Global Marketing, Solta Medical, and Gene Burdette, Director of International Marketing.

Below are some of the questions on Thermage Total Tip I’ve asked during the interview :

1. Since Total Tip is able to deliver as much as twice the bulk heating volume that delivers greater results, can you elaborate further on the greater results and how it is achieved?
The design of the tip itself allows for that nice uniform energy to be delivered across the entire treatment area, hence it provides greater heating profile, and that’s how you’re getting twice the bulk heating. The heat will build up in the centre and spread out to the edges and you get that pinching effect you felt from the original Thermage CPT treatment (Comfort Pulse Technology, 3rd generation). With Total Tip, the heating profile is very uniformed, hence heat delivery is faster and stays at the area throughout the entire treatment. This allows the collagen to be heated longer, which accounts for the immediate effect that patients will see post treatment.

2. Will one procedure be able to see the result or do we need more?
Just one procedure is needed to see results. With Total Tip, you can expect to see greater immediate results after the treatment. You can ask the doctor to treat one side of the face and proceed to do a comparison between each side of the face to see the difference.

3. Usually, how long will it take to see the result?
It takes between 8 to 12 weeks for the full results to be seen as the collagen synthesis process requires that amount of time before everything firms up completely.

4. How long will the result last?
Most doctors will say that the results last between 18 to 24 months.

5. I have learned that the more times I receive Botox injection, the longer the result will last for the subsequent treatment. Does Thermage have the same effect too?
Obviously, the more you treat and keep the collagen tight, the longer the results will last and this yields a greater effect. Some doctors recommend their patients to be on a 6-month interval, so the treatment is going to last up to 18 to 24 months. The results are stunning when patients are on a 6 to 12 month treatment cycle for Thermage. I have seen people who have kept up their Thermage treatment routine for 6 years and I can say that they look much younger at 54 than when they first started the Thermage at 48.

6. Should I have Thermage before or after Filler injection?
Usually the doctors will do the fillers first then follow by Thermage.

7. Will Thermage affect fillers?
Thermage will not affect fillers and this is what doctors like about.

8. I have tried Thermage generation 1, 2 and 3, so will the pain factor for this Total Tip be even better than generation 3 Thermage CPT Face Tip?
We are in the middle of the study now but all our doctors have been telling us that the biting effect that you experience at the back of the jawline seems to be disappearing because of the uniformity of the heating profile with the new Total Tip. So you’re not getting the spike of heat and it is much more comfortable.

9. I was told by a doctor that it is not recommended to have Thermage twice in a year, is it true?
As I’ve mentioned earlier, there are a few doctors who will do it twice in a year. It may not be necessary because you will get the effect that will last on average for a year. Doctors who treat their patients on a 6 month cycle are aiming for better results as each time you do the treatment, more of the collagen will be affected to produce the tightening effect. While it may not be necessary, some doctors believe that it doing Thermage every 6 months will produce a long-lasting effect. And I don’t see a reason why the doctor can’t give you a 6-month regimen if they wanted to.

10. I have done three Thermage procedures before and the results are not really visibly obvious. I have seen those pictures on the Thermage website and the before-and-after results are very drastic. So how many times the patients did to achieve those results?
Usually one treatment and it all depends on the patient. You will see a greater effect if you get a lot of skin elasticity loss and creases in your skin. Some patients will get better response and it also depends on what they really need.

11. Why Thermage is still the best choice for skin tightening as compared to other radio frequency machines?
Thermage has over 10 years of revolutionary technology and Solta Medical continuously refines and improves the technology to make it better for doctors and patients. We make changes to the tip and the system, to make it faster than before and more comfortable for the patients. We perfect the cooling system and are able to change the heating pattern and the profile for Total Tip. We are constantly trying to refine it, to improve patient’s comfort and to improve the speed, so it takes up less than an hour for the patient.

I don’t think there are other competitors out in the market place that have the device with the same kind of history. So we believe we have the gold standard since coming out into the market place. While others are trying to lower their cost and make treatments cheaper, if the treatment does not have the proper uniform bulk heating delivery, you’re not going to get the result. Even if you can make it more affordable for the patients, not being able to see results will be upsetting and this requires more procedures to produce obvious results. What people want is a nice single treatment product.

12. I believe some of the clinics are still using the older generation such as Thermage NXT (generation 2), will you encourage them to change to the latest?
We will definitely try to get the doctors to move to the latest technology. It hurts our reputation as the older technology is not very comfortable and to the patients, Thermage is Thermage, they don’t think about the different generations and improvement in technology and comfort level. So we are always trying to get the doctors to come onboard to our latest platform to achieve better patient outcome and to build their business with the latest technology.

During the session, I have also shared on why I like about Thermage versus the surgical facelift. To me, Thermage helps to stimulate the collagen production that gives a tighter looking skin, unlike surgical facelift that doesn’t help on collagen stimulation and worse, isn’t guaranteed to last more than 5 years. By building the collagen, Thermage helps to delay my aging process and I will age naturally.

      This new Thermage Total Tip is definitely less painful than the previous generation. I don’t feel any pain except very mild biting sensation at corner of the mouth, cheekbone and temple areas. The whole procedure is very comfortable. :D

    I did thermage CPT in March at KL after reading your blog.
    While it did tighten my skin, it doesn’t give a very lifted look esp on my cheeks.
    It also doesn’t reduce my laugh lines either. Is it supposed to be like this?
    How would rate the new thermage total tip with CPT ? While CPT is not exactly painful, it’s not comfortable either.
    I am considering neck thermage. Not sagging yet, but loose skin are definitely developing at a faster rate esp i am turning 40yrs old soon. Can share your experience ?

    PS: Nevertheless i Didnt regret my thermage treatment. The cost is definitely more worth it than the RF which is quite a waste of money. Considering mthly RF will cost abt $3K onwards for 12 session

      Hi Victoria,

      I’ve done the new Thermage Total Tip treatment less than a week now, I haven’t noticed much improvement (skin lifting) on my cheek areas yet but I do notice my nasolabiol folds are visibly reduced. My face looks slimmer and my jawline is more defined. Hence I must say the new Thermage Total Tip has shown better results than the previous tip.
